Make it happen – your decisions matter

Have you documented your future health and lifestyle wishes?
Does your family know what your wishes are?
Have you appointed someone you trust to make decisions on your behalf, if you are no longer able to make them yourself?

COTA SA, with funding from SA Department of Health, has developed the pilot Advance Care Planning Peer Education program to raise awareness and promote understanding of the importance of advance care planning. The pilot program is based in the metropolitan area of Adelaide and the target is people over 65 and/or their carers. Informative, interactive sessions delivered by trained Peer Educators are available free of charge to seniors groups, clubs and organisations.

Everyone needs to record their wishes for future health care
Communicate your wishes now to ensure the decisions you make are known
You have the right to speak up if your decisions for future care are not respected
